By completing this simple form, you can prevent problems and save lots of time and money. Read the Guidelines for more information about my requirements and processes.

I have read the Guidelines and give you permission to begin the work described above at the predetermined pay-rate.

Expect to pay a minimum of 1 hour. A 50% deposit is required for work totalling $500 or more. Rush-jobs are often billed immediately following completion. Billing typically occurs on the 1st and the 15th of every month. Cash or check is preferred. Payments not received within 30 days may be subject to a 5% late fee.
